New York State has just adopted the ACS-COT's verification process as the method it will use to verify its trauma centers. We are looking for the person who will lead us in our efforts to become verified as a Level 1 Trauma Center. Clinical duties include oversight and development of the Trauma Service as its Trauma Medical Director. You will participate as a member of the attending staff in our Surgical Intensive Care Unit and provide acute surgical care to patients admitted through our Emergency Department. Our institution is a busy, tertiary care center affiliated with the Weill Cornell Medical Center and the New York-Presbyterian Healthcare System. (Posted Feb 1, 2013) – Fellows in our program gain experience in the delivery of surgical critical care to severely injured and critically ill patients. They become competent in the application of advanced technology geared toward optimization of care for the physiologic derangements seen in critically ill patients. Fellows are also exposed to onsite instruction in the process of administration and management of critical care units, as well as contribute to the education of members of the critical care team. Fellows actively participate in our ongoing research programs. The master rotation schedule consists of 12 months. The schedule is organized so that each service acts as a building block for the next and it allows time at the beginning of the schedule for fellows to prepare for examination for certification. Fellows are on in-house call two to four nights per month. Attendings are always present in-house. Two months are reserved for elective rotations. Fellows can choose from among the following: Transplant Critical Care at the Mayo Clinic Jacksonville (4-week block) Burn Intensive Care Unit at Shands at UF (2- or 4-week block) Anesthesia at Shands Jacksonville (2- or 4-week block) Nephrology at Shands Jacksonville (2- or 4-week block) Trauma at Shands Jacksonville (2- or 4-week block) Healthcare Administration at Shands Jacksonville (2-week block) The didactic curriculum includes lectures, case discussions, and journal clubs. Topics covered include: Pharmacology/Toxicology Nutrition Cardiovascular Pediatrics Neurology Pulmonary Gastrointestinal Infectious Disease Renal Hematologic Endocrine Obstetrics The MCCKAP exam is administered annually each spring to assist fellows in preparation for the Surgical Critical Care Boards, and. to help critical care program directors to assess the strengths and weaknesses of their fellowship program and program participants. Each fellow is required to complete the exam as a preparation tool for the critical care subspecialty board examination and to identify topic areas that need further study. Fellows participate actively in ongoing research programs and are expected to bring to completion at least one project for presentation and/or publication. Fellows work with faculty as well as research coordinators who assist with the IRB approval process. Fellows must demonstrate the ability to investigate and evaluate their care of patients, to appraise and assimilate scientific evidence, and to continuously improve patient care based on constant self-evaluation and life-long learning. Fellow involvement in a quality improvement and patient safety project is required to systematically analyze practice using quality improvement methods, and implement changes with the goal of practice improvement. The UFCOM-J encourages resident and fellow research through the availability of Dean’s Fund Research Awards and an annual Research Day.

Applicants who will be considered must meet the following requirements per the ACGME guidelines: Fellows must have completed at least three clinical years in an ACGME-accredited graduate educational program in one of the following specialties: anesthesiology, emergency medicine, neurological surgery, obstetrics and gynecology, orthopaedic surgery, otolaryngology, surgery, thoracic surgery, vascular surgery, or urology. OR Fellows, who have completed an emergency medicine residency, must also complete one preliminary year of education in the surgery program at the institution where they will enroll in the surgical critical care fellowship. At a minimum the preliminary year of education must include supervised clinical experience in: pre-operative evaluation, including respiratory, cardiovascular, and nutritional evaluation pre-operative and post-operative care of surgical patients, including outpatient follow-up care care of injured patients care of patients requiring abdominal, breast, head and neck, endocrine, thoracic, and vascular operations management of complex wounds and minor operative procedures related to critical care, such as venous access, tube thoracostomy, and tracheostomy. A Pennsylvania Trauma Systems Foundation accredited level one trauma center, Hahnemann was the first trauma center in Philadelphia. We see approximately 2000 trauma patients a year and provide surgical critical care consultation on all patients admitted to the Surgical-Trauma Intensive Care Unit. The mix is approximately 85% blunt and 15% penetrating. The emergency department experiences approximately 47,000 visits annually. The ideal candidate will be trauma fellowship trained and board certified / eligible in surgical critical care. In addition, the candidate must be willing to participate in all aspects of a mature trauma program, including research, outreach and education. Hahnemann is the primary teaching site for the Drexel University College of Medicine (DUCOM), and the successful candidate will be expected to participate in the education of medical students and surgical residents. The Division of Trauma, Surgical Critical Care and Acute Care Surgery at St. Luke's University Hospital is accepting applications for its Surgical Critical Care Fellowship. The successful applicant must have completed a minimum of three years in an ACGME accredited general surgery residency. St. Luke's University Hospital is one of eleven Level 1 trauma centers in the State of Pennsylvania. The Trauma/Acute Care Surgery Service is quite active, admitting over 2300 trauma patients annually with a 94% blunt trauma and 6% penetrating trauma ratio. St. Luke’s has partnered with Temple School of Medicine to build the Regional Medical School of Temple/St. Luke’s on the campus of the Hospital. The goal of the fellowship is to develop and enhance clinical and operative skills in the complete management of critically ill surgical and trauma patients. The surgical critical care fellowship at SLUH is a newly ACGME accredited fellowship. The surgical critical care fellow is responsible for managing all patients in the 14-bed surgical/trauma ICU, which admits trauma patients, general surgery, neurosurgery, surgical oncology and other surgical sub-specialty patients. SLUH has a total critical care capacity of 38, with 20 "step down" beds, all of which the fellow will rotate throughout. The fellow will direct trauma resuscitations and resuscitations in the ICU, take trauma call, and lead a surgical critical care team of residents, medical students and advanced practitioners. The fellow will work closely with the trauma/critical care attending staff in the clinical management of critically ill patients and attending staff will also work closely with the fellow in pursuit of administrative, education and research endeavors.

The Fellowship Curriculum is composed of 9 months of critical care and 3 months of electives. Three weeks is allotted for vacation, and one week for conference attendance. Sacred Heart Hospital in Pensacola provides a 24-hour Emergency Department that includes the region's only comprehensive Trauma Center that is staffed and equipped to handle life-threatening injuries in adults and children. Sacred Heart is designated by the State of Florida as a Pediatric Trauma Referral Center and a Level II Trauma Center for adults. In 2000, the Emergency/Trauma Center was renovated and expanded. We have state-of-the art equipment and treat more than 95,000 patient visits per year. Our Trauma Team is led by trauma surgeons who coordinate care with other specialists, including neurosurgeons and orthopedic surgeons. We are looking for someone with excellent laparascopic skills. The ideal candidate will be Board-Certified in General Surgery and will have successfully completed a Trauma/Critical Care Fellowship. (Posted Feb 19, 2013) – I. Purpose of the Project The Government of Rwanda aims to build a high quality and sustainable health system. Rwanda currently faces a severe lack of adequate number of physicians and other health care workers, and one key root cause of this shortage is lack of adequate number of faculty to train future physicians. There are very few trained subspecialists, and residency training programs in Rwanda, begun in 2005, are still developing. The Ministry of Health in Rwanda developed the Human Resources for Health (HRH) Program to collaborate with US schools of medicine including: Brown, Dartmouth, Duke, Harvard, University of Colorado, University of Maryland, University of Texas, University of Virginia and Yale. The primary goal is to support clinical faculty in Rwanda and strengthen targeted clinical residency programs, with an added impact on the quality of the clinical educations for interns and medical students. As part of this program, US schools will provide expatriate physician mentors in specialties and subspecialties in the following areas: Internal Medicine, Pediatrics, Obstetrics/Gynecology, Surgery, Anesthesiology, Family and Community Medicine, Oncology, Radiology, Pathology and Emergency Medicine. The US schools that will be hiring expatriate surgeons include: Dartmouth, Harvard and University of Virginia. Expatriate surgeons with expertise in the following areas will be recruited for the Rwanda HRH Program: • General surgery (acute care/trauma) • Orthopedics/spine • Urology • Neurosurgery II. Objectives The goals of the partnership between the Ministry of Health and the affiliated US schools in the Rwanda HRH consortium include the following: 1. Support the development of the clinical, teaching, and research skills of current and future faculty and specialist physicians 2. Support the creation of a strong cadre of specialists to meet patient care needs at district, provincial and referral hospitals III. Roles and Responsibilities Each member of the US faculty for the Rwanda HRH program will be an employee of the US school, i.e. their respective “home institution” and will report to: 1) an identified mentor or supervisor at the “home institution”; and 2) the Rwanda Head of Department, an employee of the Faculty of Medicine in Rwanda. US faculty will have an appointment at their respective “home institution” and an appointment within the Faculty of Medicine. The position will pursue the following objectives: 1. To enhance the quality of education of Rwandan trainees rotating through clinical services in Surgery. The focus will be on postgraduate trainees, but will also include interns and medical students; 2. To improve quality of care of patients through role modeling, enhancement of training programs and development of evidence-based clinical care guidelines and a culture of continuous quality improvement; 3. To support those responsible for the delivery of care to patients in Surgery, for training purposes. Breakdown of specific roles for US faculty: Administrative responsibilities (20%): 1. Enhance the structure and design of clinical training, including advising on the development and implementation of administrative tools and structures required for residency operations and the creation of trainee, faculty, and program evaluations tools, including revision of curricula and modules. Educational/clinical teaching responsibilities (70-80%): 1. Actively participate in bedside-teaching, as well as seminar-based teaching 2. In collaboration with Rwandan faculty: - Participate in the development and delivery materials, programs and tools to enhance learning and teaching and provide continuing medical education - Assist in development of curriculum and pedagogical enhancements for the specific subspecialties of Surgery - Organize and conduct seminars in evidence-based medicine 3. Participate in the formal evaluation of trainees at different stages of clinical rotations to determine if they have met the requirements 4. Engage in co-mentorship with Rwanda faculty to develop clinical and teaching skills 5. Supervise postgraduate students in the delivery of care to patients for training purposes (both inpatients and outpatients) 6. In collaboration with Rwandan faculty, assist in development and/or enhancement of clinical guidelines, as well as innovative therapeutic and diagnostic approaches in Surgery • Research (0-10%) 1. Work in partnership with Rwanda counterparts to identify and develop opportunities for collaborative research. In some cases, provide training and mentorship of Rwandan faculty in research methodology. Research conducted must adhere to the ethical standards and local approval processes established by Rwandan Authorities. Research co-authored with Rwandan researchers will have shared responsibilities reflected in the authorship, and must also meet the ethical standards of the relevant US institution. IV. Qualifications • Overall Skills and Abilities: Successful candidates will have emotional intelligence and personal maturity, enthusiasm for working in a developing country, cultural flexibility and competence, and a passion for making a positive contribution to Rwanda’s healthcare delivery system. Defining characteristics are uncompromised integrity, strategic and innovative thinking skills, and the ability to foster interdisciplinary collaboration. Candidates must be able to develop and apply quantitative measurement to promote effective decision-making and continuous organizational improvement. The ability to manage multiple reporting relationships will be critical to the faculty’s success. • Communication and Reasoning Ability: Demonstrated ability to effectively and diplomatically communicate information and data in written and verbal form to top management, government officials, and hospital-based colleagues is required. The ability to interpret and apply instructions in diverse settings, and proven ability to support organizational change and develop the clinical and management capability of other health professionals is required. English proficiency required; French proficiency preferred. • Education and Experience: - Board certified or eligible in surgery - Board certified, eligible or in final fellowship year for surgical subspecialists - Professional experience in cross-cultural and low-income settings is strongly preferred V. Compensation and Benefits Salary and Benefits Salary for US faculty will be provided by the respective “home institution” and will be commensurate with experience. Medical licensure will be provided by the Ministry of Health. Housing US faculty will receive a monthly housing allowance from the Ministry of Health. In addition, they will receive an allowance to cover the first two weeks of orientation. Travel One round trip airfare ticket per US faculty and family member, as appropriate, will be provided.
